Pediatric Integrated Health Home

The Pediatric Integrated Health program (PIH) provides care coordination and family support services to Iowa children and youth who are eligible for Medicaid and have mental or behavioral health diagnosis. CHSC provides PIH services to children and families through four regional centers in northeastern Iowa: Clinton, Decorah, Dubuque, and Oelwein.

The PIH care coordination team helps children and families get the care they need by working together with the child's doctor, school, therapists, and other service providers. 

  • PIH Family Navigators have firsthand experience as the parent or primary caregiver of a child with a mental or behavioral heath challenge.
  • PIH Social Workers work with families to identify goals, develop care plans, and connect them to community-based services and resources.
  • PIH Nurses help families manage their child's medical needs.
