The Individuals with Disabilities Education Act (IDEA) requires schools to provide transition planning for all students 14 and older who receive special education services. This transition planning should be a results-oriented process andĀ focus on:
- Improving the academic and functional achievement of the student
- Facilitating the student's movement from school to post-school activities, including post-secondary education, vocational education, integrated employment and/or independent living
